Chain jumps on hills

Hello, My chain junps off the small chainring when cranking up hill in low gear?
I'll be standing up cranking pretty good all of a sudden the gears/chaing get tight and then the chain jumps off? (Shimano Ultegra) any thoughts?

Probably worn chain/cassette/chainring. What do you mean the chain "gets tight"? Maybe the derailleur is mis-adjusted and knocking the chain off to the inside?
