Rear axle help asap please

Ok I recently had my friend take my bike apart while I was at work to hurry up and get it to the powdercoater. For some reason he took the axle and everything out of the rear wheel and now I have no idea how it looked before. I put it back together the way I thought it should be and when it sits in the frame and I spin it the tire spins nice and centered at first but then it slowly makes its way over to the right until it is sitting on the frame and stops spinning. Did I build the axle and parts wrong or is this something else? I've got everything else back together but this is killing me. So if someone can help I would GREATLY appreciate it.

Front and back bearings are similar except for freewheel / hub.

Yeah Rob you are right there. I have made about EVERY mistake there is to make on this project. Pretty much my routine is I work on the bike until I get so pissed off that I want to burn it and then I jump on my road bike and take off for a long ride to cool down. At the moment I have almost everything narrowed down and at the point where I have a fixed gear BUTTTT (being the key word) I can't seem to get the chain tension right. Vertical dropouts are hell for this I know but it has to be able to be done. I am about one link away from having the correct tension so tomorrow I am off to hunt for a half link that will work with the chain. If that doesnt work its either going to be a bonfire or a VERY long ride.
